FOR IMMEDIATE RELEASE - August 27, 2009 - New York State Agriculture Commissioner Patrick Hooker today alerted consumers that Snack King Foods, Inc., located at 675 Pullman Avenue, Rochester, New York is recalling "Island Snacks Natures Mix" due to the presence of undeclared sulfites. People who have severe sensitivity to sulfites may run the risk of serious or life-threatening reactions if they consume this product.
The recalled "Island Snacks Natures Mix" is packaged in a 6.5 oz. plastic bag that is coded with the date Jul 24, 2010. The product was offered for sale at Kmart stores in Western New York State.
Routine sampling by New York State Department of Agriculture and Markets Food Inspectors and subsequent analysis of the product by Food Laboratory personnel revealed the product contained high levels of sulfites which were not declared on the label. The consumption of 10 milligrams of sulfites per serving has been reported to elicit severe reactions in some asthmatics. Anaphylactic shock could occur in certain sulfite sensitive individuals upon ingesting 10 milligrams or more of sulfites.
No illnesses have been reported to date to this Department in connection with this product. Consumers who have purchased "Island Snacks Natures Mix" should contact Brian Lauber of Snack King Foods at (585) 415-2944.

Keeping Children Safe From Sexual Predators: Child Safety & Sharing Information with Strangers
Pay attention to what your child is telling you while you are in public with him or her.
There are boundaries to which strangers are entitled to information about your child. Paying attention to what strangers are asking about your children or grandchildren is important in order to keep them safe from sexual predators. Information as to the child's age, what grade they are in and what school they go to are none of their business. It's important to realize that being protective of your chidren from sexual predators isn't acting paranoid; it's just important to teach your child that there are boundaries to which such information should be given in order to keep them safe. "Being friendly is one thing; being unsafe is another."

Lands' End Children's Light-Up Backpacks Recalled for Overheating
The following product safety recall was voluntarily conducted by the firm in cooperation with the CPSC. Consumers should stop using the product immediately unless otherwise instructed.
Name of Product: Cool Blue Backpacks
Units: About 400
Importer: Lands' End, of Dodgeville, Wis.
Hazard: The backpack's battery pack can overheat causing the battery case to melt, presenting a burn hazard if touched.
Incidents/Injuries: Lands' End has not received any reports of injuries or incidents. The hazard was discovered during internal product review.
Description: This recall involves children's Cool Blue backpacks with lights and reflective trim. The backpacks were sold in seven colors: dark blue, light blue, orange, red, green, maroon and purple. Style number 175732 is printed on a label attached to the left side of the main compartment. Backpacks sold after July 19, 2006 are not included in this recall.
Sold through: Lands' End catalog and Web site at www.landsend.com during July 2006 for about $80.
Manufactured in: China
Remedy: Consumers should stop using these backpacks immediately. Consumers with the affected backpacks are being sent a replacement backpack and battery pack with instructions to return the original product.

Quiksilver Recalls Girls’ Hoodies with Waist Drawstrings Due to Entrapment Hazard
WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed.
Name of Product: Roxy Girl “Very Nice” Cotton Hoodies
Units: About 500
Distributor: Quiksilver, Inc., of Huntington Beach, Calif.
Hazard: The cotton hoodie has a waist drawstring that could pose an entrapment hazard to children. In February 1996, CPSC issued guidelines (pdf) to help prevent children from strangling or getting entangled on the neck and waist drawstrings in upper garments, such as jackets or sweatshirts.
Incidents/Injuries: No injuries have been reported.
Description: The cotton hoodie, which zips in front and has a drawstring at the waist, came in gray with blue lining and cherry with lavender lining. They were sold in girls’ sizes SML-XL. ROXY is printed on the front of the gray hoodie and on the neck label on both colors. The style number, B487857, is printed on the inside care label near the waist.
Sold at: Nordstrom, Macy’s, Quiksilver Stores and various surf, sport, and other retailers nationwide from April 2009 through August 2009 for approximately $40.
Manufactured in: China
Remedy: Consumers should immediately remove the drawstrings from the hoodies to eliminate the hazard or return it to the place of purchase or to Quiksilver for a full refund.

WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed.
Name of Product: IKEA KARLSTAD sofa-beds
Units: About 500
Distributor: IKEA Home Furnishings, of Conshohocken, Pa.
Hazard: The mattress and seat cushions intended to be used as a mattress fail to meet the mandatory federal open flame standard for mattresses, posing a fire hazard to consumers.
Incidents/Injuries: No injuries were reported.
Description: IKEA KARLSTAD sofa-bed frame has article number 801-215-12, supplier number 20789 with date stamps (YYWW) 0845 through 0927 located on a label attached to the underside of the sofa-bed frame. The mattress topper is not affected by this recall.
Sold at: IKEA stores nationwide from November 2008 through July 2009 for about $850.
Manufactured in: Mexico
Remedy: Consumers should immediately stop using the mattress and cushions and contact IKEA or visit the Returns and Exchange Department at their local IKEA store to arrange free installation of a replacement mattress and seat cushions.

Name of Product: Force Soldier Playsets, Pirate Expeditions with Parrot, and Pirate Expeditions with Treasure
Units: About 8,400
Importer/Distributor: Liquidation Outlet, Inc., of Lakewood, Wash.
Hazard: The action figure toys have surface paints which contain excessive levels of lead, violating the federal lead paint standard.
Incidents/Injuries: No injuries were reported.
Description: The Force Soldier playsets comes in a plastic bag with SKU number 70134 printed on the header card. The soldiers (two per bag) are 5” in height and come with an airplane, machine guns and a tropical tree. The Pirate Expeditions come in a blister packs in two styles with the SKU number 70136 printed on the header cards. Pirate Expedition figures are 6.5” in height and have assorted accessories including a parrot or a treasure chest.
Sold at: Dollar Stores in Washington and Oregon States from September 2007 through July 2009 for $1.
Manufactured in: China
Remedy: Consumers should take the recalled products away from young children immediately and return them to the place of purchase for a full refund or exchange for another toy.

WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed.
Name of Product: Certain Frigidaire and Kenmore Elite Smoothtop Electric Ranges
Units: About 200,000
Manufacturer: Frigidaire, a division of Electrolux Home Products Inc., of Augusta, Ga.
Hazard: Depending on the model, the surface heating elements can: 1) turn on spontaneously without being switched on; 2) fail to turn off after being switched off; or, 3) heat to different temperatures than selected. This poses a fire and burn hazard to consumers.
Incidents/Injuries: Frigidaire has received 126 reports of incidents, including four reports of minor burns and two reports of minor property damage.
Description: This recall involves Frigidaire, Frigidaire Gallery, Frigidaire Professional and Kenmore Elite smoothtop electric ranges with rotary knobs and digital displays. The following model and serial numbers are included in the recall:

Baby Jogger Recalls Strollers Due to Fall Hazard

Hazard: The stroller’s restraint buckle could break or unlatch allowing the child or infant to fall out.

Description: This recall involves Baby Jogger City Mini single and double strollers. “City Mini” is printed on the stroller. Item numbers and date codes included in this recall are listed in the chart below along with stroller colors. The item number is printed on a sticker on the rear wheel crossbar. The manufacturing date code is engraved on the frame behind the rear wheel axle crossbar or on the back of the frame on the upright bar between the right rear wheel and the seat.

Texting While Driving - Video [Warning - Very Graphic]
WARNING: EVERY driver should see this, especially teenagers. It is very graphic and is recommended for 18 and older but if they wait till 18 to see it, they may not reach 18. PARENTS: Watch this by yourself first then WITH your kids. Explain to them that if they HAVE TO phone or text, pull over and stop in a SAFE location first, THEN use the phone.
Texting while driving is an extremely dangerous endeavor.

Over 91 Children Have Died Since 2000 Due to Strangulation as a Result of the Use of Blinds Even with Today's "Safer Blinds"

A number of recalls have been issued regarding the use of blinds due to strangulation which include a number of the "safer" blinds. This video needs to be heeded as to risk and hazards to children where these products are installed where children reside or play. The solution is to install "cordless" window shade products.

Near Strangulation of Children Prompts Recall of Roman Shades; Sold Exclusively at Pottery Barn Kids
WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed.
Name of Product: Roman Shades
Units: About 85,000 units
Distributor: Pottery Barn Kids/Williams-Sonoma, Inc. of San Francisco, Calif.
Hazard: Strangulations can occur when a child places his/her neck between the exposed inner cord and the fabric on the backside of the blind or when a child pulls the cord out and wraps it around his/her neck.
Incidents/Injuries: There have been six reports of children becoming entangled in the exposed inner cord on the back of the shade between March 2006 and July 2008. Four children were found with cords entangled around their necks. Scissors were used to release two of these children. Two children were found with red marks around their necks after having freed themselves. No permanent injuries were sustained.
Description: This recall involves all sizes and colors of Pottery Barn Kids roman shades with exposed inner cords on the back. Two sewn-in labels are located at the bottom edge of the shade. The smaller label identifies the manufacturer as “pottery barn kids” while the second larger orange, black and white label warns that “Cords and bead chains can loop around child’s neck and STRANGLE.” Roman shades with a fabric covering over the cords are not being recalled.
Sold at: Pottery Barn Kids catalog nationwide and through the firm’s Web site at www.potterybarnkids.com from January 2003 through May 2007 for between $30 and $60.
Manufactured in: China and Hong Kong
Remedy: Consumers should immediately stop using the recalled roman shades and contact Pottery Barn Kids to receive a merchandise card for the purchase value.

Risk of Strangulation Prompts Recall to Repair Roller Shades by Lutron Shading Solutions
WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed.
Name of Product: Roller Shades
Units: About 245,000
Distributor: Lutron Electronics Co. Inc., of Coopersburg, Pa.
Manufacturer: Virginia Iron and Metal Company Inc. (VIMCO), of Ashland, Va.
Hazard: Strangulations can occur if the shade’s looped bead chain is not attached to the wall or the floor and a child’s neck becomes entangled on the free-standing loop.
Incidents/Injuries: None reported.
Description: This recall involves custom-ordered made-to-measure manual roller shades. The shades were sold in a variety of colors, fabrics, and sizes. They have a label on an adhesive strip that connects the shade fabric to the roller tube that reads “Lutron Shading Solutions” and “800-446-1503.” The shade needs to be removed from the brackets and unrolled completely to access the label. The shade was sold with an orange, black and white hang tag warning consumers that “young children can STRANGLE in cord and bead chain loops.” However, the hang tag may have been discarded upon installation. Nearly all shades were professionally installed.
Sold at: Specialty dealers and Expo Design Centers nationwide from January 2000 through April 2009 for between $200 and $2000 per shade.

Remedy: Consumers should immediately check their window coverings to see if the tension device is attached to the cord and installed into the wall or floor. If not attached, immediately stop using the window coverings and contact Lutron Electronics to receive a free repair kit. If the consumer has difficulty installing the tensioning device, contact Lutron for further instructions.

Strangulation Death of a Child Prompts Recall To Repair Window Blinds By Vertical Land
WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed.
Name of Products: Horizontal and Vertical Blinds and Cellular Shades
Units: About 15,400 (horizontal), 16,400 (vertical), 800 (Cellular Shades)
Manufacturer: Vertical Land Inc., of Panama City Beach, Fla.
Hazard: Horizontal Blinds: The blinds do not have inner cord stop devices to prevent the accessible inner cords from being pulled out. If an inner cord is pulled out, a child can become entangled in the loop and strangle.
Vertical Blinds: Strangulations can occur if a child’s neck becomes entangled on the free-standing loop formed by the bead chain or by the cord with a weighted device.
Cellular Shades: Strangulations can occur if a child’s neck becomes entangled on the shade’s free-standing looped cord.
Incidents/Injuries: In May 2006, CPSC received a report of a 4-year-old girl from Pensacola, Fla., who strangled in the loop of a vertical blind cord that was not attached to the wall or floor.
Description: This recall involves all Vertical Land custom-made vertical and cellular window coverings that do not have a cord tensioning device and horizontal blinds that do not have inner cord stop devices. A sticker identification label can be found on the head rail of some of the window coverings which lists the company’s name, address and telephone number.
Sold at: Vertical Land stores in Panama City and Pensacola, Fla. from January 1992 through December 2006 for between $60 and $200 (depending on the size of the blind).

Remedy: Consumers should immediately stop using the window coverings and contact Vertical Land to receive a free retrofit kit.

WASHINGTON, D.C. - The CPSC and Goodman Manufacturing Company, of Houston, TX, are urging consumers who have not yet responded to the previously-announced 1998 recall to do so immediately. After October 1, 2009, the remedy consumers receive will change to be identical to modifications recently announced for a related recall.
The recall included about 10,000 Plexvent and Ultravent HTPV pipe systems attached to certain Goodman mid-efficiency furnaces manufactured from October 1988 to July 1994 (models GUP, GDP, GUPS, GDPS, GUPI, GDPI, GUPX, GDPX, GMP and GMPV for the following brands manufactured by Goodman: Janitrol, GMC, Hamilton Electric, Franklin, Liberty and Sears/Kenmore). The HTPV pipe used for these vents could be susceptible to corrosion, cracking and joint separation, which could result in the release of carbon monoxide (CO) into living areas, posing a danger to consumers. Goodman did not manufacture the HTPV pipe.
The following table describes the different remedies available to consumers with qualifying furnaces that use the two leading brands of HTPV pipe – Plexvent and Ultravent.

Valid claims initiated on or before October 1, 2009, with remediation completed and required documentation submitted by January 1, 2010. Will receive
a new, professionally-installed venting system free of charge
or
a new, high-efficiency Goodman furnace and suitable vent for the manufacturer's price (of just the furnace), with no charge for labor, associated materials or dealer markup.
Valid claims initiated after October 1, 2009 A rebate up to $400 toward either an HTPV replacement system, or a new, high-efficiency furnace from Goodman that does not require HTPV. A rebate up to $250 toward either an HTPV replacement system, or a new, high-efficiency furnace from Goodman that does not require HTPV.
Consumers who register after October 1, 2009 and who choose to repair their systems will be responsible for up-front payment of parts, labor and permits, and will be responsible for arranging to have the work performed.
Consumers should determine whether they have a recalled HTPV pipe system by checking the vent pipes attached to their natural gas furnace. Vent pipes subject to this recall can be identified as follows:
1. the vent pipes are plastic;
2. the vent pipes are colored gray or black;
3. "Plexvent," "Plexvent II" or "Ultravent" is stamped on the vent pipe or printed on stickers placed on pieces used to connect the vent pipes; and
4. the vent pipes are located on furnaces and the pipes go through the sidewalls of structures.
Other plastic vent pipes, such as white PVC, are not included in the recall.
Owners of Goodman furnaces that are vented with HTPV pipe should immediately call Goodman at (800) 394-8084 from 8 a.m. to 4:30 p.m. CT Monday through Friday. CPSC reminds all consumers to have fuel-burning appliances professionally inspected each year to check for cracks or separations in the vents that could allow CO to leak into the home. In addition, CPSC recommends that every home should have at least one CO alarm.

Pella Recalls Casement Windows Due to Impact Hazard
The following product safety recall was voluntarily conducted by the firm in cooperation with the CPSC. Consumers should stop using the product immediately unless otherwise instructed.
Name of Product: Architect Series® and 850 Series® Casement Windows
Units: About 6,500
Manufacturer: Pella Corp., of Pella, Iowa
Hazard: The hinge can break, causing the window sash to fall out, posing a risk of injury to persons beneath the window.
Incidents/Injuries: The firm is aware of one report of the hinge breaking, causing the sash to fall. No injuries have been reported.
Description: This recall involves the Architect Series® and 850 Series® casement windows. The windows were sold in a variety of sizes and typically contain a heavier window glaze such as the HurricaneShield® Impact-Resistant glass.
Sold by: Authorized distributors nationwide and Home Improvement Stores in Florida from September 2007 through April 2009 for between $500 and $2000.

Remedy: Consumers should immediately stop using the window and contact Pella to schedule a free repair. Owners have received direct mail notification of this recall.

Ameriwood Recalls Entertainment Stands Sold Exclusively at Walmart Stores Due to Tip-Over Hazard
The following product safety recall was voluntarily conducted by the firm in cooperation with the CPSC. Consumers should stop using the product immediately unless otherwise instructed.
Name of Product: Your Zone Loft Collection Entertainment Stands
Units: About 195
Manufacturer: Ameriwood Industries, of Wright City, Mo.
Hazard: Warning labels sold with the entertainment stands indicate incorrect size and weight limits for televisions used with the stands. A television that is too heavy or wide for the entertainment stand can make the unit unstable, posing a tip-over hazard to consumers.
Incidents/Injuries: None reported.
Description: This recall involves Your Zone Loft Collection entertainment stands. The stands are intended to be used as gaming centers. The recall includes Autumn Beech YZ084-104-13-19 stands with model number 1156020WY. Model information is printed on the instruction manual.
Sold exclusively at: Walmart stores nationwide from May 2009 through July 2009 for about $100.
Manufactured in: Canada
Remedy: Consumers should immediately check the size and weight of the television used with the entertainment stands. If the television exceeds 50 pounds or does not sit entirely on top of the stand, consumers should immediately remove it from the stand. CRT televisions larger than 20 inches and flat panel televisions larger than 27 inches should not be placed on the stands. Consumers should contact Ameriwood for a replacement warning label.

Maytag Expands Recall of Refrigerators Due to Fire Hazard
WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product.
Name of Product: Maytag®, Magic Chef®, Performa by Maytag® and Crosley® brand refrigerators
Units: About 46,000 (1.6 million units were previously recalled March 2009)
Manufacturer: Maytag Corp., of Newton, Iowa
Hazard: An electrical failure in the relay, the component that turns on the refrigerator’s compressor, can cause overheating and pose a serious fire hazard.
Incidents/Injuries: Maytag has received 23 additional reports of refrigerator relay ignition, including 4 reports of property damage ranging from smoke damage to extensive kitchen damage.
Description: The recall includes certain Maytag®, Magic Chef®, Performa by Maytag® and Crosley® brand side-by-side and top freezer refrigerators. The affected refrigerators were manufactured in black, bisque, white and stainless steel. They have model and serial numbers printed on a label located on the top middle or left upper side of the refrigerator liner and have the following model and serial number combinations:
